When to use

Use during launch planning, renewal review, governance review, issue cleanup, or when a risk needs routing without building a task system.

Who should use this

Operations lead, advisor, compliance reviewer, HR, finance owner, governance owner, or HRB designee.

Review boundary

This tool identifies and routes risk. It does not make legal, tax, fiduciary, payroll, Medicare, carrier, or compliance determinations.

Risk category

  • Classify the risk, such as financial, legal, tax, employee experience, vendor, evidence, or operating risk.
  • Flag sensitive or qualified-review topics.
  • Avoid treating the category as a determination.

Risk description

  • Describe the issue in plain language.
  • Separate known facts from assumptions.
  • Record source or signal.

Likelihood and impact

  • Use simple low/medium/high language if helpful.
  • Identify who or what is affected.
  • Avoid quantitative modeling unless separately documented.

Owner

  • Name the routine owner or role.
  • Identify any owner gap.
  • Do not create live assignments or due dates.

  • Name legal, tax, compliance, finance, Medicare, COBRA, vendor, or other review owner where needed.
  • Document the review boundary.
  • Escalate unresolved review needs.

  • Define the operating action, evidence action, communication action, or review action.
  • Keep mitigation practical and specific.
  • Connect to related toolkit resources.

Escalation trigger

  • Define what causes escalation, such as missing owner, time-sensitive issue, coverage risk, payment risk, privacy issue, or unresolved review.
  • Name escalation owner.
  • Retain escalation evidence.
